I, the undersigned, agree to indemnify and hold harmless the Lupus Foundation of America, Greater Florida Chapter, Tropicana Field and Tampa Bay Rays from all cost, expense and liability arising out of my or my child’s participation in Walk For Lupus Now events including but not limited to: falls, contact with other participants, weather, traffic and conditions of the field. I do hereby waive all claims for damage or loss to me or my child's person or property which may be caused by an act, or failure to act, by the aforementioned entities, their officers, agents or employers arising directly or indirectly from my or my child's participation in this event; and I hereby assume liability for any loss, damage or other liability from such event. I understand that I should not enter this walk unless I am medically able. I agree to abide by any decision of the event officials relative to my ability to safely complete the course. I hereby grant permission to LFAGF and its authorized agents to use my name and photographs, videotapes, motion pictures, or recordings or any other record of my participation in this event for any purpose. I further understand that there will be a $20 fee for all returned checks and that this event will take place rain or shine—no refunds will be issued.
